All registration forms must be filled out completely and submitted with a non-refundable $100 registration fee.

This form is to be filled out and signed by child's physician.
This form is to be filled out and signed by child's parent. This gives us a general overview of child's individual health, toileting, and snack routines and preferences directly from you, the parent.
Please fill out this form to give LLL permission to photograph your child during special events
In the unlikely circumstance that Little Lamb Learners Preschool is instructed to evacuate by emergency personal the children will be transported to the Lower Macungie Library located on Brookside Road to await pick up by the parent, guardian, or other individual designated by the parent/guardian.  In this form you provide alternate contacts that the child  may be released to other than the parent/guardian. 
This form acknowledges your receipt of all necessary forms and paperwork, as well as certifies your agreement to follow the policies and procedures at Little Lamb Learners, detailed in our Family Handbook. Family Handbooks are available as hardcopies only, and will be given out during Meet the Teacher. If enrolling after Meet the Teacher for that school year, you will receive a handbook upon registration.
Please fill out Emergency Information (brightly colored) card given to you during Registration. We will provide this card to you during Meet the Teacher or on child's first day.  If for some reason you do not have your card, please use the link above and fill it out prior to your child's first day. Staff may still ask you to copy some of the information over to the card.
